Visiting Angels is seeking a compassionate and detail-oriented Registered Nurse (RN) to join our Bridgeport, WV team.
The RN is responsible for completing comprehensive client assessments, developing individualized care plans, monitoring client health needs, and ensuring all services meet regulatory, safety, and quality-of-care standards. The RN works closely with caregivers, clients, families, and office staff to support continuity of care for our elderly population.


Principal Accountabilities and Essential Functions:
The Registered Nurse has accountability for the following functions/tasks:

Client Assessments & Care Planning:
  • Conduct comprehensive initial assessments for clients in their homes to determine physical, cognitive, emotional, and environmental needs.

  • Develop individualized care plans based on assessment findings to ensure safe, appropriate, and high-quality care.

  • Perform reassessments at required intervals or when changes in condition occur, updating care plans accordingly.

  • Identify risks, safety concerns, and care needs, documenting recommendations for both clients and caregivers.

  • Evaluate caregiver performance from a clinical standpoint and ensure care plans are being followed appropriately.

Clinical Oversight & Monitoring:
  • Provide ongoing clinical support and oversight for clients receiving home care services.

  • Monitor changes in client condition and communicate promptly with caregivers, office staff, and families.

  • Ensure all care delivered is compliant with Visiting Angels policies, state regulations, and nursing best practices.

  • Provide training, coaching, or instruction to caregiving staff regarding client-specific care needs when required.

  • Consult with the leadership team on high-risk or complex cases to ensure proper care coordination.

Documentation & Compliance:
  • Maintain accurate and timely documentation of all assessments, reassessments, home visits, care plan updates, and clinical notes.

  • Ensure all records meet state, federal, and company documentation standards.

  • Participate in compliance audits and support regulatory preparedness.

  • Maintain knowledge of state home care regulations, clinical standards, and safety protocols.

Communication & Client Relationships:
  • Build strong, supportive relationships with clients, families, caregivers, and referral sources.

  • Educate clients and family members on care needs, safety measures, and health management strategies.

  • Respond professionally and calmly to urgent client situations or unexpected changes in condition.

  • Collaborate closely with the scheduler, office staff, and management to ensure continuity of care.

  • Display strong organizational skills and the ability to manage multiple assessments and visits efficiently.

  • Maintain regular and predictable attendance.

  • Present a well-groomed, professional image consistent with Visiting Angels standards.
     

Required Job Knowledge and Skills:
  • Current and active Registered Nurse (RN) license in the state.

  • Minimum one year of clinical nursing experience; home care, geriatric, or community health nursing strongly preferred.

  • Proficiency in Microsoft Office applications and electronic charting/documentation systems.

  • Strong written and verbal communication skills, with the ability to communicate with empathy and clarity.

  • Ability to listen effectively and engage with clients and their families.

  • Reliable transportation and ability to travel to client homes.

  • Must successfully complete all background checks, drug/alcohol testing, health screenings, and compliance requirements.
     

Physical/Environmental Demands:
  • Must be able to lift, push, or pull up to 10 pounds.

  • Must be able to perform home visits involving walking, standing, bending, reaching, and navigating stairs.

  • Must be able to see and hear or use prosthetics enabling adequate performance of nursing duties.
